Ensuring that all referrals are approved according to the patient referral criteria established by the mission.
Evaluate each referral proposal and validate according to agree upon criteria.Ensuring the management of patients referred.Together and in coordination with the project team and the logistics department, organizing the movement of patients, guards and accompanying medical staff (when necessary).Referring/accompanying the patient to the most appropriate hospital, according to the patient's pathology and the competencies/specialties of each of the selected structures and according to the mission's referral policy, to ensure the best possible care.Ensuring regular monitoring of the patient's progress, with regular visits to the health facilities, participation in ward rounds of our patients, and discussions, if necessary, with the staff, making sure that the treatment is given and the diagnostic tests are performed within the established criteria;Ensuring that MSF patients do receive timely treatment (are not being kept unnecessarily) not hang around in the chosen hospitals for no logical reason to avoid additional costs for MSFInforming the project medical management in case of difficulties in management.Taking care of administrative issues related to referrals (completion of patient files, counter-referrals, per diem, invoice control, appointments, etc.).Ensuring the regular update of the referral table of the project and send them on a weekly basis for a good follow-up of the evolution of referred patients.Ensuring that the counter-referral file is updated in the project once the patient is discharged from the health facility.MSF focal point in the hospitals where the patients are referred, were the interlocutor with the nursing staff (nurses, doctors, surgeons) and administrative staff of the health structures involved.If necessary, supporting the department in other areas related to patient management (sending samples, tracking specific drugs with the Ministry of Health and others).ContextShubra clinic is a partnership program with Egypt without diseases (EWD), based only on a referral basis/appointments with no walk in consultations. MSF will be providing technical supervision and guidance via the Project medical Coorindator (PMC) and all the available teams and resources of MSF.
As such, number of patients per day is limited to the following factors with a poyential evolution of the project if needed.Laboratory capacity inside the clinic will focus only on essential point of care diagnostics.
All additional investigations will be referred to an external laboratory.
The clinic will not have a pharmacy/dispensary.
The doctors shall prescribe the medications to the patients who will be directed to an external pharmacy that will dispense the medication according to an agreement made with the clinic.
Beside above accountabilities, as the only MSF nurse in the clinic, this position is also resposible to: Collaborate closely with the Social Worker and other multidisciplinary team members to assess, identify, and prioritize patients' social and medical needsCoordinate appropriate medical referrals and support-service access, and work with the Social Worker to facilitate social work, NGO, and external social support referrals to ensure comprehensive, patient-centered care.Collaborate with EWD medical team for the clinic medical referrals, also ensure the pharmacy and labroatary order from MSF/EWD clinic is according to MSF protocols and guidelines.Collaborate with the Health Promotion team in clinic for handling the Hotline and referrals in to this clinic.Assist the management team for the strategies and scopes of clinic according to patients data and conditions.According to Operational needs, and upon respecting the communication lines, might be requested to assist other nurses related duties within MSF-EWD Clinic, Responsible for filing MSF doucment, mantainance of MSF equipment and belonging.Report any concerns, barriers, or system weaknesses to the line manager, propose feasible solutions, and contribute to the continuous improvement of the referral and clinic workflow, medical and clinical pathway.
Register and gather proper patient data for managers in the clinic for strategies for next phase.Establish and maintain an effective referral system, ensuring streamlined procedures, timely case management, and close coordination with project teams and external referral stakeholders.
Maintain updated mapping of available formal and informal support services (including community-based resources) and assess their quality and accessibility.
Report identified gaps and suggest options to fill them.
Ensure effective communication and collaboration with service providers, medical focal points, and partners assisting populations, to maintain a functional and coordinated referral network.
Gather and document case-related information using standardized tools and the database developed by the documentalist , ensuring confidentiality and compliance with data protection protocols.
Track patients' attendance at scheduled appointments and referral services, and support adherence through reminders and accompaniment when requiredMaintain an updated defaulter tracking system to identify, document, and escalate missed appointments to the HPO for timely and appropriate follow-up and intervention.Ensure confidentiality of the cases, and all their related information;
Participate in case discussions to improve the processes of the comprehensive approach.
Supporting in implementation of safeguarding polices and create a safe space to our beneficiaries.
Perfom other tasks as per request.
Education Essential: Registered nurse or other health specialization Desirable: Degree in social work Experience Essential: Minimum 2 years’ experience working in healthcare.
Desirable: Experience working in social work and experience working with vulnerable populations.
Experience with MSF or other NGOs.
Experience of collaboration with Ministry of Health institutions.
